MONTHLY PAYMENTS
Claim for Monthly Payments
VA Form 29-4125a
Use this form to receive your life insurance benefit as monthly installment payments
instead of a lump sum. Choose from fixed-term or lifetime payment options.
- Option 2: Fixed monthly payments (36–240 months)
- Option 3: Lifetime payments, 120 guaranteed
- Option 4: Lifetime payments equal to policy face amount
- Proof of age required for Options 3 & 4

If you prefer a steady monthly income stream over time, use VA Form 29-4125a.
